My signature below indicates that if I am approved for funding, I acknowledge that I will be held accountable for class attendance and study hall requirements as determined by academic staff. Failure to meet all academic requirements will result in the cancellation of scholarship funding. Additionally, if I am approved for funding and drop courses, fail courses, stop attending class/academic commitments or violate any Athletics Department policy, I will lose access to future funding opportunities and will be financially responsible for the term in which the courses are dropped or failed.
